The official German theory test (TÜV/DEKRA) is available in 12 languages apart from German:

Is it harder in English?
No. The questions are direct translations of the official German questions. However, the translations can sometimes be slightly clunky or formal ("bureaucratic English").
Does it cost extra?
Generally, no. The official exam fee (approx. €25) is the same regardless of language. Some driving schools might charge a small administrative fee for booking an "English slot," but this is rare.
Recommendation
If your German is not at a native or near-native level (C1+), take it in English. The questions can be tricky and contain "double negatives". Don't add a language barrier to a logic test.
